Ordering Information

There are two ways you can go about ordering printed or electronic resources from the Healthy Start Clearinghouse:

Visit our Online Catalog, add items to your cart, and it will guide you through the ordering process. This site will compile your order form and on the last page, you must print it out and either postal-service-mail it to us with your payment, or fax it to us. Your order is not complete until we receive your order via fax or postal service.
Download an order form in Adobe Acrobat PDF format, print it out, and fill it out by hand. Then, you can either fax it to us or mail it through the postal service.

We accept Checks, Money Orders, Visa, MasterCard, and POs.
Minimum credit card order is $20 including shipping and tax.

Shipping cost is $5.00 plus 10% of your order subtotal.
